He was predeceased by: his parents, Hobert Perkins and Margaret Perkins (Smith); his spouse Shirley Curl; and his sons, Mark A. Perkins and William A. Perkins.
He is survived by: his children, Andrew Perkins (Rita), Bonita Burroughs, Shaylon Martinez (Mario) and Francis Perkins (Melissa); and his brother David Perkins (Robin). He is also survived by 9 grandchildren, and 6 great-grandchildren.
In lieu of flowers, memorial donations in Andy's honor, may be made to the United Cerebral Palsy Foundation.
