He is survived by: his wife Phyllis A. Forst of Lake Milton; his children, Lisa Leyman of Newton Falls, Jamie White of Newton Falls, Jessica Hileman (Jason) of Howland and Jason White of Lake Milton; and his siblings, Patricia, Linda, Darlene, Deborah and Philip Leyman.
