He is survived by: his girlfriend Dolly Smith; his children, Jacob Sylvia (Siera), Brittany Burns, Shane Warner, Jonny Ray Smith, Nathan Warner, Ryan Warner and Elizabeht Warner; and his brother Kenneth James Sylvia (Stacey).
