He was predeceased by: his wife Juanita Ely (Music); and his daughter-in-law Tina Kodak. He is survived by: his children, Rick Kodak, David Kodak and Tracy Shafer. He is also survived by 4 grandkids; many great-grandkids; 1 sister, and 2 brothers.
