Koch, Jr. and Mary Kathleen Spyker; his children, Dakota Allen Koch of Lima, Destiny Marie Koch (Dakota Grim) of Lima and Alexis Taylor Koch (Brady Navarro) of Lima; his grandchildren, Mayleigh Sue Marie Grim and Leah Jane Grim; and his siblings, Wendie Marie Haley (Adrian) of Springfield, IL, Christa Dawn McGuire (Brian) of Waynesfield and Gene A. Koch, III (Samantha Miller) of Lima.
Memorial contributions may be given to Cleveland Clinic Children's Hospital.
