He was predeceased by: his parents, Rudolph A. Kubasak and Hellen Kubasak (Stacko); and his niece Tammy Kubasak. He is survived by: his wife Judy G. Kubasak (Waldecki); his sons, Todd Kubasak (Denise) of Lorain and Mark Kubasak (Heather) of Mount Perry, Ohio; his grandchildren, Mason, Blake, Bella and Chase Kubasak; his brothers, Rob Kubasak (Debbie) of St. Augustine and Rich Kubasak (Kathy) of Lorain, Ohio; and his stepsisters, Vicki Radican and Christine Koba. He is also survived by many nieces and nephews.
