She was predeceased by: her parents, Raymond C. Mulac and Ada E. Mulac (Moskin); her former husband Michael Checkosky, Sr.; and her close friend Susan. She is survived by: her son Michael Checkosky, Jr. of Macedonia; her grandson Jackson Checkosky; her brother Dennis Mulac (Sharon) of North Olmsted; her sister-in-law Mary Kay Slaby (late Tom); and her nieces and nephews, Lori, Jeff (Yan), Lindsey, Christopher, Michelle, Geoff and Greg (Jackie).
In lieu of flowers, Susan requested that donations be made in her memory to the church.
