He is survived by: his wife Evelyn Collins; his siblings, Jim Collins, Vicki Collins, Darryl Collins and Scott Collins; his siblings-in-law, Greg Blythe, Michelle Amyx, Lonnie Blythe (Alice) and Laura Cook (Jack); his nieces and nephews, Joshua Blythe, Angie Blythe, Amber Baughman (Aaron), Adam Blythe (Jamie) and Aric Blythe (Liz); and his great-nieces and great-nephews, Kinsley Blythe, Bennett Blythe, James Blythe, Isaiah Blythe (Veda) and Olyvia Blythe.
Visitation will be held on Friday, June 20th 2025 from 10:00 AM to 11:00 AM at the Well of Grace Pentecostal Outreach (4263 OH-350, Lebanon, OH 45036). A memorial service will be held on Friday, June 20th 2025 at 11:00 AM at the same location.
