Phillips (Hafner); his wife Andrea Phillips; his children, Nevaeh Phillips, Lynn Daniels (Billy) and James Phillips; his granddaughters, Loreeanah and Breeanah Daniels; his sisters, Michelle Frank, Marsha Cutlip (Jeff), Monica DeWitt (Mark) and Megan Phillips; his father-in-law Duane Sours; and his brother-in-law Joe Massie (Tina).

Visitation will be held on Wednesday, November 4th 2026 from 2:00 PM to 3:00 PM at the Ohio Cremation & Memorial (9 Chambers Rd, Ontario, OH 44906). A memorial service will be held on Wednesday, November 4th 2026 at 3:00 PM at the same location.
