He is survived by: his children, Nicole Foulks-Carman (Jay) of Mansfield, OH, Brittany Foulks-Joseph (Tim) of Lakewood, OH, Brandyn Foulks (Melanie) of Smithville, OH and Adam Foulks (Quay Koontz) of Lawton, OK; his grandchildren, Alex and Megan Foulks; his son-in-law Dan Cornell; and his siblings, John Pate (Jennifer), Jeffrey Pate, Brian Foulks, Kenny Foulks (Cheryl), Charlotte Smith, JoAnn Winters and Jean Foulks. He is also survived by many nieces, nephews, other family and friends.
