He was predeceased by: his sister Melody. He is survived by: his wife Dawn (Sam); his children, Elishia (Mark) and Eric (Kasey); his siblings, Wendy, Susan, Lisa and Jeff; and his stepmother Dorothy. He is also survived by numerous nieces and nephews.
