He is survived by: his parents, Sam Slater and Debbie Slater; his sister Megan (John Dickhaus); and his nephews, Cayse and Colby Dickhaus. He is also survived by many loving uncles, aunts, cousins, and friends.
In lieu of flowers, the family asks that donations be made to the Mason Youth Organization, the Humane Association of Warren County, or to a local youth sports organization or animal shelter in Dan's memory.
