He was predeceased by: his parents, Lucille Ostapack (Capaldi) and Walter Ostapack; and his brother Christopher Daniel. He is survived by: his siblings, Toni of Canton, Patty of Albuquerque, Doug (Ann) of Horse Cave, Mark (Christine) of Massillon, Ohio, Alan, Scott (Joyce) and Nick (Mary Ann) of Albuquerque; his nieces and nephews, Jarrett, Jacob, Sean, Reese, Ted, Angela, Crystal, Maggee, Rachael and Mallory; and his great-nieces and great-nephews, Zephyr, Hudson, Oliver, Jack, Olivia, Kendel, Rylan, Aryanna, Remi, Haley and Stella.
The family requests in lieu of flowers donations be made to the Ohio.
