He was predeceased by : his parents, Leonard Michalak and Carolyn Michalak (Veitch); and his stepfather Harry Krempa. He is survived by : his wife Marie Michalak; his daughters, Kelly Fitzgerald, Kristie Guilford (Jason), Pamela Michalak (Eric Stewart) and Kimberly Gloeckl (Bryan); his grandchildren, Jacob Guilford, Ryan Guilford, Nick Guilford, Tyler Barton (Gabby Adams) and Megan Barton (Tommy Kegler); and his brother Jack Michalak (Sue).

Memorial donations in Tom's name may be made to the American Heart Association.
