He was predeceased by: his parents, Jan Dziak and Aloyzia Dziak; his daughter Angelina Mays; his great grandchild Nyah; and his sisters, Karoline, Ruzena, Alojzia and Czechoslovakia.
He is survived by: his daughters, Rozena, Ashley and Vera; his grandchildren, Amanda, Gabrielle, Aiden, Avarie and Arlo; and his brothers, Jozef, Cyril and Metod.
