He was predeceased by: his parents, Alvin Ballard and Ruby Ballard; and his son-in-law Mitch Cieslak. He is survived by: his children, Timothy Ballard of Hamilton and Susan Cieslak of Spring City, Tennessee; his grandchildren, Rachel Agee (Matt), Chris Cieslak (Julia), Jenna Combs (Jesse), Brittney Ponchot (Brayden) and Miranda Ballard; his great grandchildren, Maddie Agee, Luke Agee, Aiden Womacks, Jace Schneider, Ashden Womacks, Alden Womacks, Heidi Cieslak, Ben Cieslak and CeCe Cieslak; and his close friend Joanne Rickard.
Visitation will be held on Monday, October 26th 2026 from 11:00 AM to 1:00 PM at the Baker-Stevens-Paramore Funeral Home (6850 Roosevelt Ave, Middletown, OH 45005). A celebration of life will be held on Monday, October 26th 2026 at 1:00 PM at the same location.
