It is with great sadness that we announce the death of Sally Ann Sparks of Middletown, Ohio, born in Hamilton, Ohio, who passed away on September 16, 2023, at the age of 87, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Herrick Polen and Martha Polen (Scott); her husband John Sparks; and her brothers, Wendell Polen (Wendy) and Richard Polen (Dick).
She is survived by: her daughters, Nancy Murrell (Larry) and Sharon Sparks; her granddaughter Sarah; and her great grandchild Charlotte. She is also survived by several cousins, nieces and nephews, and many friends.
In lieu of flowers, donations can be made to Hospice of Cincinnati who with great dedication, lovingly cared for Sally in her final days.
