He was predeceased by: his parents, James Cook and Anna Kiser; his son Robert "Bob" (Joann); and his sister Cheryl Remley. He is survived by: his wife Donna; his children, David Jr. (Leah), Stacey Donaghy (Shawn) and Shannon Cook; his grandchildren, Danielle, Cody, Blake, Trent, Heidi, Josh, Braden, Mark and Shawn McKenna; his great grandchildren, Tristen, Cash, River and Bentley; and his brothers, James (Sharon) and John (Bev).
In lieu of flowers, donations can be made to the American Heart Association or American Cancer Society in David's honor.
