He is survived by: his children, John "Jay" Jr. of Toledo, Cole of Columbia and Abigail of Bowling Green, Ohio; his parents, Richard Pollex and Darla Pollex; and his siblings, Joe, James (Shannon) and Kathleen (Monem). He is also survived by his nieces and nephews.
