He is survived by: his wife Jeanette Lynn Forst (Justice); his daughter Tina Chappell (Dan) of New Alexandria; his stepchildren, Lori Pensis (Mikey) of Columbus, Sonya Pensis of Columbus and Zachary Taylor of Steubenville; and his grandchildren, Chelsee Russell (T. J.) of Pickerington and Gino Barber of Pittsburgh.
