He was predeceased by: his parents, Frederick E. Mayhew and Elaine Mayhew (McElroy); and his grandmother Mary McElroy (O'Bryant). He is survived by: his wife Carol Mayhew (Smith); his children, Fred Mayhew (Melissa) of Harrisonburg, Virginia, Courtney Smith (Mark) of Fredericktown and Jim Daubenspeck (Jennifer) of Meredith; his grandchildren, Miles Mayhew, Sadie Mayhew, Bryant Smith (Lauren), Logan Smith (Kristen), Jerica Smith (Brandon Joyner), Meghan Daubenspeck, Nate Daubenspeck and Derek Daubenspeck; and his great grandchildren, Lane Smith, Luke Smith, Colt Smith, Amaiyah Smith, Isaiah Smith, McKinley Joyner and Kohen Joyner.
