He was predeceased by: his parents, Glenn Lewis and Gladys Lewis; and his brother Glenn "Butch" Lewis. He is survived by: his spouse Gayle; his sister Sheila Snyder; his siblings-in-law, Kaye Lewis, David and Susan Brobst; and his caregivers, Kelly Benjamin, Heidi Bowman and Emily Wilson. He is also survived by numerous nieces and nephews, and many friends.
