He was predeceased by: his parents, Frank Kosik and Wilma Kosik (Sheak); and his brother Jimmy. He is survived by: his wife Cindy Kosik (Gonser); his stepdaughter Annie Shankle (Matt); and his siblings, Tom Kosik of Newark, Steve Kosik of Colorado, Sue Sennhenn of South Carolina and Nancy Striegl of Wisconsin. He is also survived by several nieces and nephews.
