He was predeceased by: his parents, Louise Collins and John A. Collins. He is survived by: his sons, Andrew (Maria) of Cross River, NY, Stuart (Korby) of Rocky River, OH and Christopher (Theresa) of Evanston, IL.; and his siblings, Mary Green of Chandler, AZ and James Collins of Oro Valley, AZ. He is also survived by seven grandchildren, three great-grandchildren, and many dear friends.
