He was predeceased by: his mother Juanita Hall. He is survived by: his father Eddie Cook of Norwalk, Ohio.; his son Michael Cook (Tara) of Cleveland; and his grandson Jacob. He is also survived by many other aunts, uncles, and cousins.
Eddie's final act of kindness was to give the gift of life through organ, eye, and tissue donation.
