We announce the death of Phyllis A. Perry of Norwalk, Ohio, who passed away on August 28, 2026, at the age of 86. She was predeceased by: her parents, Peter Sklarek and Carol Sklarek; her son Dale Perry; her husbands, Ray, Russ and Richard; her brother Ken Sklarek (Mary); and her brother-in-law Ronald Kuzak.
She is survived by: her son Rick Perry (Heather); her grandchildren, Isaac Perry (Corynne), Alexa Bristol (Aaron), Mitchell Perry and Jared Perry; her daughter Kathleen Holtz (Daniel); her grandchildren, Brooke and Evan; her daughter-in-law Wanda Perry; and her sister Patricia Kuzak. She is also survived by nieces and nephews.
A funeral Mass was held on Friday, September 4th 2026 at 11:00 AM at the St. Paul Catholic Church (91 E Main St, Norwalk, OH 44857).
