She was predeceased by : her mother Teresa "Sissy" Holbrook (Miller); her uncle Gregory Miller; and her grandparents, Vernie and Kathleen Miller. She is survived by : her children, Rachelle Lewis, Colton Lewis and Savannah Lewis; her father Michael Holbrook Sr. (Pam) of Waterloo, Ohio; her siblings, Holly Farwick (Andrew) of Sardinia and Michael Holbrook Jr. of Waterloo Ohio; her grandparents, John and Deanna Holbrook of Salyersville, Kentucky; her aunts and uncles, Johnny Holbrook of Waterloo, Cindy Holbrook of Dayton, Debbie Reynolds of Concord, Steve Miller of Patriot and Vernon Miller of Grayson, Kentucky; her nieces and nephews, Rosalynn, Adeline, Josephine Farwick, Claudia Holbrook and Michael Holbrook III; and the father of her children Billy Lewis.

In lieu of flowers, memorial donations may be made to the funeral home to help with expenses.
