He was predeceased by: his parents, Marion Kozlowski and Stanley Kozlowski; his brother-in-law Frank Rork; his son-in-law Max Morrissey; John and Jean Hatfield.
He is survived by: his significant other Janelle Bihn; his daughter Jillian Cordes (Josh); his siblings, Jim Kozlowski (Mickey) and Mary Beth Rork; and his nephews, Buck Kozlowski (Ohamy), Jimmy Kozlowski (Colleen) and Danny Rork (Emma). He is also survived by 7 great nieces and nephews, 5 "grandchildren" and best friend.
