He is survived by : his children, Steven Puthoff (Lanita), Michael Puthoff (Deborah), Jennifer Setser (Richard), Paul Puthoff (Julie) and Mary Beth Calendine; his grandchildren, Andrew, Samantha, Alixandra, Miranda, Christopher, Nathaniel, Jacob, Joshua, Connor, Molly and Nicholas; and his siblings, Robert "Duke", Suzanne (Rod), Richard (Stephanie), Joanne, Roger (Debbie), Victoria (Michael), Raymond (Linda) and Gregory (Tammy).

In lieu of flowers, memorial donations may be made to the St. Louis Church or the St. Louis School.
