He was loved and cherished by many people including: his parents, Sonnie Jean Faga and Rev. Robert O. Former; his children, Ryan and Kelly (Bryan); his grandchild Morgan Bailey; and his siblings, Cyndi Ladd (Don), Becky Jarvis (Dave), Gayle Haller (Tim) and Marla Rievley (Eddie).
In lieu of flowers, memorial donations in David's name may be made to Royal Redeemer Church or the Wounded Warrior Project.
