He was predeceased by: his parents, George W. Hoffman and Pearl Hoffman (Ludwig); his wife Phyllis A. Smith; his siblings, George J. "Junior" Hoffman, Basil Hoffman, Violet DePew, Dorselene Gunderman and Deloris Stoller; and his pet Oscar.
He is survived by: his daughters, Denise Renollet (Duane) of Cecil, OH, Linda Miller (William) of Spencerville, IN and Jackie Hanlon (Bryan) of New Haven, IN; his sister Patricia Arend (Ivan) of Paulding, OH; his grandchildren, Stephanie Renollet, Lisa Bidlack, Jared Renollet, Brittany Fleckenstein and Andrew Miller; and his pet Brutus. He is also survived by 7 great grandchildren.
Visitation will be held on Wednesday, July 2nd 2025 from 11:30 AM to 1:30 PM at the Den Herder Funeral Home (1000 W Wayne St, Paulding, OH 45879). A funeral service will be held on Wednesday, July 2nd 2025 at 1:30 PM at the same location.
Memorial contributions may be made in Jack's memory to the Paulding County Dog Shelter.
