He was predeceased by: his sister Vicki Sprow; his mother-in-law Susan LaBounty; and his grandparents, Richard, Christine Zuber and Erma Smith.
He is survived by: his wife Ashley Smith (LaBounty); his children, Lailah, Sophia, Adriana and Caroline; and his parents, Jerry Smith (Brenda) and Linda Zuber. He is also survived by five sisters, nieces, nephews, many extended family members, and friends.
Memorial contributions may be made in Sam's memory in care of his wife, Ashley Smith, for his daughters' education expenses or to the Paulding County Dog Shelter.
