He is survived by: his wife Debra; his children, Chad (Lauren) of Fort Mill, Nathan of Powell, Ohio and Alexa Mendleson (Brian) of Piqua, Ohio; his grandchildren, Bridgette McDaniel, Owen Wortman, Landen Wortman, Katie Wortman and Brooks Wortman; and his siblings, Amy Opp (Glen) of Colorado, John Wortman (Brenda) of Wapakoneta, Jim Wortman (Joan) of Findlay, Sarah House (Bryan) of Boaz and Paul Wortman (Maureen) of Findlay, Ohio. He is also survived by many nieces and nephews.
