He is survived by: his children, Ken McIntosh of Pleasant Plain, Amanda Starr of Middletown, Stephanie McIntosh of South Lebanon and Melyssa McIntosh; and his son-in-law Rei Diaz. He is also survived by 11 grandchildren and 17 great-grandchildren.
