He was predeceased by: his parents, Thomas Thomas and Flora Faye Thomas; and his siblings, Mary Hill, Louis, Paul and James Thomas.
He is survived by: his sons, Ben (Lisa) of Shawnee Hills and Luke (Christy) of Scottsdale, Arizona.; and his grandchildren, Morgan Jack (Colin) and Collin Thomas (Melinda). He is also survived by uncle to his nieces and nephews.
In lieu of flowers, memorial contributions in David's honor may be made through the OhioHealth Foundation , or to a cancer care organization of the donor's.
