She is survived by: her significant other Dennis Evans; her daughter Megan Frank; her grandchildren, Brycyn and London; her siblings, Michael Westlake and Cindy Robinson; her nieces and nephews, Chris Robinson (Trish), Zachary Westlake (Sarah), Jeremiah Westlake (Holly), Nikki Perkins (Carl) and Samantha Westlake; her pets, Ruby and Tilly; and her mother-in-law Margaret Evans.
Memorial donations in Jackie's honor may be made to the Hardin County Council on Aging.
