He was predeceased by: his parents, Jewel Kowalski and Raymond Kowalski; and his sister Belinda Markey Dietrich (Kowalski). He is survived by: his sons, Jeremy and Nathaniel Kowalski; and his sibling Mickey Rose (Doug).

Per last wishes, in lieu of flowers, David has requested donations be handled by sibling Patricia (Kowalski) Ehrnsberger.
