He was predeceased by: his father George Loftin; his wife Patricia Ann Lantz; and his siblings, Carleen Kirkman and Tremaine Fields. He is survived by: his children, Jennifer Hawa (David) of Hampstead, NC, Jerry Fields, Jr. (Kris) of Delta, OH and Jon Fields (Monique) of Sandusky, OH; his grandchildren, Alec Hawa, Taylor Kopocs (Jacob) and Jordyn Waldron (Shawn); and his sisters, Maxine Thornlow and Katherine Miller. He is also survived by his wife and family, having 3 children.
