She was predeceased by: her former husband Richard Forsthoffer. She is survived by: her husband Mark; her children, Tylor Oravec (Sydnie Wolny), Tiffany Malkowski (Danny) and Jacob Forsthoffer; her grandchildren, Ryan Forsthoffer, Gabrielle Malkowski and Lilian Malkowski; and her siblings, Mike Charek, Steve Charek and Sue Windbigler.
Visitation was held on Monday, November 3rd 2025 from 10:00 AM to 11:00 AM at the St. Martin of Tours Catholic Church (1800 Station Rd, Valley City, OH 44280). A Mass was held on Monday, November 3rd 2025 at 11:00 AM at the same location. A burial was held on Monday, November 3rd 2025 at 1:45 PM at the Ohio Western Reserve National Cemetery (10175 Rawiga Rd, Seville, OH 44273).
