He is survived by: his wife Kim; his children, Robert D. Slack (Jessica), Michael Slack, Stephanie Tracy (Chris) and Michelle Koger (Chris); his grandchildren, Tony (Abby), Gavin, Hunter, Ava, Addison, Alena, Landon and Ellie; his siblings, Drew Smith, Doug Lowe (Debbie), Becky West (Tim), Diane Sheeley (Tom), Tim Slack (Terry), Eugene McCormick, Peggy Slack, Deb Less (Bob) and Sandi Walterbusch; and his mother-in-law Barbara McCormick.
