He was predeceased by: his parents, Leroy Smith and Elsie Smith (Petitjean); his brothers, David and Paul; and his sisters-in-law, Mary and Pat.
He is survived by: his wife Helen Jean Smith (Howard); his stepdaughter Karen Burchfield (Gary); his former wife Doris A. Smith; his children, Donna Muldoon (Dave), Steve Smith and John Smith (Laura); his brother Robert; and his sister-in-law Marla Wagner. He is also survived by 6 grandchildren, 3 great grandchildren, numerous nieces and nephews.
In lieu of flowers, memorial contributions may be made in his name to the Ohio Masonic Home Foundation.
