He was predeceased by: his parents, William R. Worthington and Patricia Worthington (Payne); his wife Judy Worthington; and his siblings, Pat, Terry Worthington and Valerie Pape Wilson.

He is survived by: his children, Guy Jr. and Andrea; his siblings, Gale Pape, Rick Worthington, Darren Worthington, Cindy Duetscher, Tina Briggs and Debbie Worthington; his niece Kim Wilson; and his caregivers, Barbara Edwards, Sonny Stewart and Paul Wilson. He is also survived by several grandchildren, numerous nieces, nephews, & a host of friends.
