He was predeceased by: his aunt Marcia Reed; and his close friend Jack. He is survived by: his wife Kay; his sons, Jay R. Cook, Jr. (Michelle Boylan) and Jason Cook (Heather); his brother John Cook (Lori); and his grandsons, Brendan Cook and Mason Sowards.
