He was predeceased by: his daughter Gracelin Fails. He is survived by: his parents, John Fails (Brenda) and Christina Powell-Jenkins of Greenfield Ohio; his siblings, Amber Beverly and Sam Fails; his daughters, Tatum Martin, Lynsie Martin and Eden Fails; his grandson Maverick Fails; and his aunts, Melissa Winters (Mark) and Catharine Fails.
