He was predeceased by: his parents, John Kulisek and Christine Kulisek (Jandik). He is survived by: his wife Janice Kulisek (Coder); his children, Vanessa Ferrari (Chris), Susan Phillips (Del) and Jonathan Kulisek; his grandchildren, Anna and Alex Ferrari; and his sisters, Ellen and Christine Kulisek. He is also survived by several nieces and nephews.
