He was predeceased by: his wife Janice Jacobs. He is survived by: his children, Jenifer Maddox, Jill Finegan and John Jacobs, Jr. (John Sr.); and his brother Michael Bralek. He is also survived by six grandchildren, and two great-grandchildren.
