She is survived by: her children, Tom Forman (Leslie), Nancy Forman Beers, Scott Forman (Ronda), Mark Forman (Pam) and Amy Forman Hill; her grandchildren, Kevin Beers (Selby), Benjamin Forman (Kristin), Karen Beers Horvath (Andy), Dan Forman (Kaley), Rachel Forman, Sarah Forman Rothhaar (Joel), Gregory Beers (Christine), Abby Forman Salge (Lou), Joel Forman (Carolina), Lauren Forman, Andrew Beers (Caitlin), Mary Forman Sherman (Brennan), Wyatt Hill (Ashlee), Wyatt Uresti (Kerri), Paul Forman (Hannah), Hayden Hill and Gracelyn Decker; her siblings, Joanne Walterhouse (Dale), Susan Creamer (Dale) and Richard Klenk (Adele); and her friends, Din Milem and Joann Spangler. She is also survived by 28 great-grandchildren.
Visitation was held on Thursday, September 10th 2026 from 11:00 AM to 1:00 PM at the Sunbury United Methodist Church (100 W Cherry St, Sunbury, OH 43074). A funeral service was held on Thursday, September 10th 2026 from 1:00 PM to 1:45 PM at the same location. A graveside service was held on Thursday, September 10th 2026 from 2:00 PM to 2:15 PM at the Sunbury Memorial Park (44 W Cherry St, Sunbury, OH 43074).
