He was predeceased by: his parents, John Joseph and Mary Ellen Levy. He is survived by: his wife Patricia A. Levy; his children, Erin Mianegaz (Omid), Nicholas Levy (Anne) and Ana Carmichael (Jeff); his grandchildren, Mya, Emma and Avery Mianegaz; his siblings, Paul Levy (Susan), Carol Piening (Steve), Mark Levy (Sue) and Dan Levy (Sue); and his siblings-in-law, Peg Roudebush (late Bud) and Jim Griebel (Diane). He is also survived by nieces, nephews, cousins, as well as many dear friends.
