He was predeceased by: his parents, Ralph Converse Cook and Cleo Cook (Near). He is survived by: his wife Linda Cook (Wilkins); his sons, Chris (Kim) and Matt Cook (Kendra); his grandchildren, Landon, Evan, Lauren and Charlie; his brother Mike Cook (Annabelle); his nephew Aaron; and his nieces, Kristie and Rachel.
In lieu of flowers, the family kindly requests that donations be made to the Perry County Cancer Alliance, P.O. Box 724, New Lexington, Ohio 43764. in remembrance of Clark's life and in support of a cause dear to his heart.
