He was predeceased by: his parents, Errold M. Cook and Alice Cook (Ebersole); his spouse Rella E. Collins; and his siblings, Jack Cook, Jerry Cook and Doris Rose.
He is survived by: his children, Michael Cook (Deb) of Bettsville, Steven M. Cook (Mindy) of Tiffin and Brenda S. Brandaberry of Fostoria; and his brother Harold "Hank" Cook of Fostoria. He is also survived by six grandchildren; and 13 great grandchildren.
